Home Health Physical Therapist

LHC Group is seeking a skilled Physical Therapist to join our team in Eupora, Mississippi. As a Physical Therapist with LHC Group, you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their goals.
About the Role:- Assess and evaluate patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
- Develop and implement treatment plans to improve and enhance patient well-being.
- Evaluate patient progress and report to the supervising nurse and/or physician.
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
- Instruct and inform participating members of the healthcare team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
- Current Physical Therapy licensure in the state of practice.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
