About the Role
The Registered Nurse (RN) will provide high-quality care and services to patients across various settings, including private homes, skilled nursing facilities, and assisted living facilities. As a vital member of the interdisciplinary team, you will contribute your expertise in psych-social assessments, interventions, and community resources. You may work in Case Management, Admissions, or Resource roles, ensuring seamless collaboration with healthcare professionals.
Requirements
* Current RN licensure (in good standing) in the relevant state(s)* Collaborate effectively within a team environment to support daily operations* Develop strong relationships with patients while receiving support from a dedicated patient care team* Demonstrate independent work skills, prioritizing tasks such as phone calls, email correspondence, meetings, and documentation* Manage time efficiently to meet visit schedules, attend required meetings, and respond to urgent needs* Learn and adapt to electronic medical records systems* Possess reliable transportation, a valid driver's license, and auto insurance to travel across the service area*
