About the Role

We are seeking a compassionate and skilled Home Health Aide to join our team at Hackensack Meridian Health. As a Home Health Aide, you will provide high-quality, personalized care to patients in the comfort of their own homes.

Responsibilities
  • Provide in-home care services to patients, including personal care activities such as bathing, dressing, and grooming.
  • Assist patients with daily living tasks, such as meal preparation, light housekeeping, and medication management.
  • Monitor patients' conditions and report any changes to the RN or other healthcare professionals.
  • Document patient care activities and progress in a timely and accurate manner.
  • Assist patients with transfers and ambulation, and perform range of motion exercises as needed.
  • Perform general housekeeping duties, including changing bed linens and cleaning patient areas.
  • Assist patients with self-administration of oral medications and remind them to take their medications as directed.
  • Prepare simple meals and assist patients with eating as needed.
  • Assist with ostomy care, including changing colostomy bags and performing peri care.
Requirements
  • Home Health Aide certification
  • Valid driver's license from a USA state
  • Good customer service skills
  • Ability to lift a minimum of 50 pounds and stand for 8 hours a day
