Job Title: Private Duty Nurse LPN

Job Summary:

Aveanna Healthcare is seeking a compassionate and skilled Private Duty Nurse LPN to provide high-quality care to our medically fragile patients in their homes. As a Private Duty Nurse LPN, you will be responsible for implementing physician-prescribed plans of care, conducting patient assessments, and administering prescribed medications and treatments.

Responsibilities:

  • Implement physician-prescribed plans of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medications and treatments
  • Document care provided in system of record
  • Teach and train family members on health promotion and disease prevention
  • Accept and write verbal orders from physicians as required
  • Follow up with physician, client, family members, and staff on client problems and concerns

Requirements:

  • Active, unencumbered LPN license in the state of practice
  • CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred
  • Reliable transportation
