Key Responsibilities:
  • Provide personalized care to patients, adhering to all applicable laws, regulations, and policies
  • Focus on the medical needs and treatment of patients, following an active plan of care
  • Assess and document patient observations at each visit
  • Conduct patient evaluation tasks, including reviewing medication and vital signs
  • Administer prescribed medication, treatments, and therapies
  • Coordinate care with physicians, PT, and other individuals in the patient's care plan
  • Provide education to patients, caregivers, and family members
Requirements:
  • Active RN/LPN License to practice in the State of Florida
  • One year of clinical-based experience in home health, preferred
  • Current CPR certification
  • Self-driven and accountable for providing excellent patient care and attention to detail
  • Ability to deliver skilled services, including assessment and skilled observation, wound care, and dressing changes
  • Maintain a level of knowledge to navigate electronic medical records systems
