Key Responsibilities:
  • Conduct initial and ongoing assessments to evaluate patient mobility, coordination, strength, and current level of function.
  • Develop and implement personalized plans of care to achieve patient goals and outcomes.
  • Deliver physical therapy services in accordance with prescribed physician orders and maintain accurate documentation.
  • Collaborate with healthcare team members to ensure seamless patient care and effective communication.
  • Supervise and educate patients and their caregivers on home exercise plans and discharge care.
Requirements:
  • Master's degree in Physical Therapy from an accredited university/program.
  • Current, unrestricted state license as a Physical Therapist in the state of practice.
  • Valid CPR certification.
Preferred Qualifications:
  • Strong organization and communication skills.
  • Attention to detail and time management skills.
  • Proven relationship-builder with experience in Home Health Therapy and OASIS documentation.
