Join Our Team as a Physical Therapist - Home Health Specialist

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team as a Home Health Specialist. As a Home Health Specialist, you will be responsible for providing high-quality physical therapy services to clients in their homes, promoting independence and improving their overall health and well-being.

Responsibilities:
  • Conduct home visits to assess clients' physical therapy needs and develop personalized care plans.
  • Implement evidence-based interventions to improve clients' mobility, strength, and balance.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Provide education and training to clients and their families on physical therapy techniques and strategies.
  • Document client progress and outcomes, and participate in quality improvement initiatives.
Qualifications:
  • Current licensure as a Physical Therapist in the state of New Hampshire.
  • Minimum of one year of experience as a Physical Therapist in a home health setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
  • Ability to work independently and manage time effectively, with a focus on delivering high-quality care.
