Clinical Care Coordinator

We are seeking a skilled LPN to join our team as a Clinical Care Coordinator. In this role, you will be responsible for overseeing the clinical care of our elders, working closely with the Clinical Support Team to ensure the highest level of care.
Key Responsibilities- Provide clinical care to elders in accordance with state and federal guidelines, nursing standards, and Maple Crest policies.
- Obtain daily updates on each elder's condition and provide a daily nursing report.
- Visit each elder during each shift to monitor their clinical status and complete clinical assessments.
- Administer and document treatments and medications as needed.
- Work collaboratively with the Elder Assistant, ALC, and Clinical Support Team to ensure seamless care.
- Current LPN license in the State of Ohio.
- Ability to recognize and respect the autonomy and dignity of all elders.
- Excellent communication and customer service skills.
- Ability to work independently and as part of a team.
