Role Overview

The Faith Community Health Coordinator position at Franciscan Health is designed to support the Faith Nursing Program. This role collaborates with senior leaders to set priorities and develop strategies for advancing the Faith Nurse program within the Franciscan Health system and in the larger community.

Key Responsibilities

  • Create and steward collaborative interdepartmental and community relationships to leverage partnerships and resources for community health.
  • Participate in meetings, serve on committees, and represent the department in community outreach efforts.
  • Coordinate all aspects of the Faith Nurse program, including educational opportunities, meetings, and resource support.
  • Support community initiatives to promote collaborative arrangements among Franciscan Health, community partners, and general community members.
  • Attend conferences and meetings as the hospital's representative and share findings with leadership.
  • Coordinate community benefit reports and ensure accurate reporting for government and community benefit reporting.
  • Assist with the Community Health Needs Assessment and Community Health Improvement Plan.

Qualifications

  • Bachelor's Degree in Public Health or Allied Health Profession certificate required.
  • Bachelor's Degree in Nursing or Health Care related preferred.
  • 3 years of working with patients and/or community members in a health/public health setting required.
  • 1 year of working with community organizations preferred.
  • 3 years of Community or Faith Nursing preferred.
  • RN, LPN, CMA, AAMA, or EMT preferred.
  • Valid Driver's License required for local travel and occasional travel to other Franciscan regions.
