Key Responsibilities
  • Conduct thorough assessments of patients' medical needs and develop comprehensive care plans to address their unique requirements.
  • Provide skilled nursing services, including wound care, medication management, and education to patients and their families.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure seamless care coordination and optimal patient outcomes.
  • Monitor patients' progress, adjust care plans as needed, and communicate effectively with patients, families, and healthcare teams.
  • Supervise and train Home Health Aides and other support staff to ensure high-quality care delivery.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
Requirements
  • Diploma, Associate, or Bachelor's Degree in Nursing
  • Minimum one year of nursing experience, preferably in home health or a related setting
  • Strong medical-surgical, ICU, ER, and acute care experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Excellent organizational, communication, and interpersonal skills
  • Valid driver's license, auto insurance, and reliable transportation
