Job Opportunity: Physical Therapist - Home Health Care Specialist

Job Summary:

We are seeking a skilled and compassionate Physical Therapist to join our team at Jefferson Health at Home by BAYADA. As a Physical Therapist, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities:

  • Conduct thorough assessments and develop personalized care plans to meet the unique needs of each patient.
  • Perform diagnostic tests and measurements to evaluate patient mobility, strength, and balance.
  • Implement evidence-based interventions, including manual therapeutic exercises, gait training, and balance therapy.
  • Collaborate with the multidisciplinary care team to ensure seamless patient care and achieve optimal outcomes.
  • Provide education and training to patients and their families on proper techniques and strategies for maintaining independence and preventing complications.

Requirements:

  • Active and current license to practice as a professional Physical Therapist in the state of PA.
  • Previous home healthcare experience is a plus, but not required. We offer comprehensive training and support to ensure your success.
  • Ability to travel to patient homes as assigned.
  • Excellent communication and interpersonal skills, with a calm and reassuring presence.
  • Strong assessment skills and ability to deliver patient care with compassion, excellence, and reliability.
