We are seeking a skilled Physical Therapist to join our team at AccentCare, Inc. as a Home Health Physical Therapist in El Centro, CA.
Key Responsibilities- Provide high-quality physical therapy services to patients in their homes, promoting independence and improving overall health.
- Conduct thorough assessments, develop and implement individualized treatment plans, and evaluate patient progress.
- Collaborate with healthcare teams, including physicians, nurses, and other therapists, to ensure comprehensive care.
- Communicate effectively with patients, families, and caregivers to educate them on treatment plans and promote adherence.
- Maintain accurate records, documentation, and billing in accordance with company policies and regulatory requirements.
- Master's degree in physical therapy from an accredited program.
- Current licensure to practice as a physical therapist in the state of California.
- Minimum nine months of experience as a physical therapist, with preferred experience in community or home health settings.
- Valid CPR certification and a reliable vehicle for travel to patient locations.
- A competitive salary range of $102,000 to $160,000 per year.
- A $10,000 bonus for experienced candidates.
- A comprehensive benefits package, including medical, dental, and vision coverage, paid time off, and professional development opportunities.
- A $5,000 relocation package for eligible candidates.
