Job Summary

We are seeking a skilled and empathetic Hospice Registered Nurse/ RN Case Manager to join our team. As a key member of our interdisciplinary team, you will provide direct patient care services, support patients and their families, and collaborate with our team to develop and implement personalized care plans.

Responsibilities
  • Provide direct patient care services, including assessing patient needs, developing and implementing care plans, and evaluating patient progress.
  • Act as a liaison between physicians, patients, and families, ensuring seamless communication and coordination of care.
  • Collaborate with our interdisciplinary team to develop and implement personalized care plans, addressing the physical, emotional, and spiritual needs of patients and their families.
  • Empower patients' caregivers by educating them on care for their loved ones, promoting independence and confidence.
  • Manage and coordinate the care of patients, including evaluating patient needs, developing care plans, and collaborating with our team to ensure timely and effective care.
  • Maintain accurate and comprehensive patient records, ensuring compliance with regulatory requirements and company policies.
  • Participate in quality assessment and performance improvement initiatives, contributing to the continuous improvement of our care services.
Requirements
  • RN license in the state of employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
  • Additional experience in hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing
  • Current driver's license and automobile insurance
  • Current CPR certification
