Job Summary

We are seeking a skilled Housing Navigator to join our team at Adapt Integrated Health Care. The successful candidate will be responsible for connecting clients with housing options, providing case management assistance, and collaborating with community partners to address housing needs and barriers.

Key Responsibilities
  • Housing Navigation: Collaborate with community housing partners, property management companies, and private landlords to identify and secure short-term and long-term housing options for clients.
  • Case Management: Provide assistance with housing applications, benefits, and other paperwork to secure housing for clients.
  • Collaboration: Work effectively with the court system and treatment teams to identify and address housing needs and barriers.
Requirements
  • Education: Bachelor's degree in a related field, such as social work or psychology.
  • Experience: Two years of experience in a social service or mental health setting, or a satisfactory equivalent combination of education and experience.
  • Skills: Knowledge of basic psychological theory, principles, and practices of mental health treatment; human behavior as it relates to crisis situations and problems of client population; substance abuse treatment as it pertains to the mentally ill; considerable knowledge of behavior intervention, management, and support strategies; sound case management; knowledge of community resources, including the functions and procedures of other social service agencies and service providers.

  • Competitive Salary: $45,000 - $61,000 per year.
