About the Role:

Vitra Health is seeking a compassionate and skilled Registered Nurse to join our team in the Brockton area. As an In-Home RN, you will play a vital role in providing high-quality care to our clients in the comfort of their own homes.

Key Responsibilities:
  • Conduct regular in-person visits with clients and caregivers to ensure their safety and well-being.
  • Collaborate with the Care Manager to review and upload Care Communication Tools (CCT) at the time of each visit.
  • Evaluate potential clients, assess their needs, and complete all necessary documentation.
  • Provide ongoing training and support to caregivers on health, aging, and related topics.
  • Complete nursing progress notes and other required documentation.
  • Work collaboratively with the interdisciplinary team to create and implement client-centered Plans of Care.
Requirements:
  • Licensed by the Massachusetts Board of Registration in Nursing.
  • At least two years of recent experience in the direct care of elders or adults with disabilities.
  • Excellent interpersonal and communication skills.
  • Valid driver's license with proof of insurance.
  • Bilingual in English and secondary language preferred.
