Job Title: Home Health Physical Therapist

Supplemental Health Care is seeking a skilled Physical Therapist to join our team for a travel assignment in Bloomington, Indiana.

Job Description:

We are looking for a Physical Therapist with prior OASIS experience to work at a partnering home health care provider in Bloomington, Indiana. As a Physical Therapist with Supplemental Health Care, you will have the opportunity to work with a reputable home health care provider and make a positive impact on the lives of our patients.

Contract Details:
  • $2,020 – $2,188 per week*
  • 13-week contract
  • Paid by the hour – not by the visit
  • Physical Therapist will have a designated territory of coverage with a set amount of patients
  • Home Health contracts with SHC require previous experience with OASIS/Medicare Certified Visits
  • *Estimated weekly payment may include both taxable wages and tax-free reimbursements for meals, housing, and incidentals. Please speak with a recruiter for details.
Requirements:
  • Graduate of an accredited PT program.
  • Licensed as a Physical Therapist in Indiana
  • CPR Certified or ability to obtain
  • Previous experience with Computer Charting
  • 6 months of previous Home Health Care experience
  • Valid U.S. Driver's License and reliable transportation
