Job Summary:
Amedisys Inc is seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Hospice Nurse - On-Call Monday-Friday 4p-12a. As a Hospice Nurse, you will provide patient-centered care to individuals in need of hospice services, working closely with our interdisciplinary team to ensure the best possible outcomes.
Responsibilities:
- Perform initial nursing assessments on new admissions to identify patient needs and develop individualized care plans.
- Collaborate with the clinical manager, physician, and care team to ensure continuity of patient care and make referrals to other hospice care disciplines as needed.
- Communicate patient progress to the clinical manager, physician, and care team, and maintain accurate and timely patient care progress records.
- Provide education to families on patient condition, needs, and care, and make referrals to other hospice care disciplines as needed.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
Requirements:
- Current RN license, specific to the state(s) you are assigned to work.
- One+ year of nursing experience required.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
