Job Summary

We are seeking a skilled Licensed Practical Nurse to join our Family Practice team. As a key member of our healthcare team, you will provide high-quality patient care and support our medical staff in delivering exceptional patient outcomes.

Key Responsibilities
  • Administer medications and immunizations to patients, ensuring accurate dosages and adherence to medical protocols.
  • Provide education to patients and their families on health-related topics, promoting informed decision-making and self-care.
  • Assist with ordering and maintenance of clinical supplies, onsite medications, and equipment, ensuring a well-stocked and efficient clinical environment.
  • Support the administration of the Virginia state vaccine program by performing vaccine inventory and assisting with orders.
  • Collaborate with the multidisciplinary team to provide coordinated and patient-centered primary care, including medical providers, behavioral health providers, family support workers, referral coordinators, and medical assistants.
  • Respond to patient phone calls, questions, and requests for care, working closely with the medical patient care team to ensure timely and effective support.
  • Inform patients of lab and diagnostic results, and facilitate follow-up care as needed.
  • Assist patients in navigating the healthcare system, working closely with referral coordinators to ensure seamless transitions and optimal outcomes.
  • Participate in clinic quality improvement and risk management activities, including individual patient outreach, population health campaigns, and chronic care management.
  • Track patients who require follow-up GYN procedures and diagnostic tests, coordinate receipt of results, and assist patients with scheduling procedures off-site.
  • Fulfill medical assistant duties as needed, ensuring a smooth and efficient clinical environment.
  • Prepare and maintain special internal and external reports as requested by the Supervisor and/or Department Head.
  • Assist with developing, coordinating, and recommending changes for the improvement of workflow in the program or section area.
  • May be assigned to various project lead roles to advance the development of the department and support ad hoc needs.
Requirements
  • Active Licensed Practical Nurse license in Virginia.
  • 1+ years of experience as a Licensed Practical Nurse.
  • 1+ years of experience working with specialty in Pediatrics, Family Medicine, Women's Health.
  • 1+ years of experience with healthcare insurance.
  • Strong organizational and communication skills.
  • Working knowledge of Microsoft Office and Microsoft Excel.
  • Fluency in Spanish highly preferred.
  • Clinical competency in the administration, storage, and handling of vaccine services.
  • Ability to work effectively with a diverse workforce and patient base.
  • Ability to work in a fast-paced environment.
  • BLS/CPR required.
