Job Summary

We are seeking a highly skilled Clinical Care Coordinator to join our team at BayMark Health Services. As a Clinical Care Coordinator, you will play a critical role in ensuring the maintenance of quality care and monitoring the treatment of all patients.

Key Responsibilities
  • Ensure the maintenance of quality care and monitor treatment of all patients.
  • Conduct internal program review, patient records review, and educational counseling related to substance abuse disorders.
  • Assess training needs of clinical and support staff and report to the Program Director.
  • Ensure staff compliance with Federal and State regulations governing the treatment of patients.
  • Attend conferences, meetings, and training programs as needed.
  • Provide intake/readmission services and follow-up calls to all discharging patients for at least 30 days.
  • Provide crisis intervention/supportive services to patients as needed.
  • Perform individual and peer file reviews on a monthly basis for quality control.
  • Participate in or direct community and public relations activities, including advertising and marketing.
Qualifications
  • LADC with a Master's degree in behavioral health, and/or LCMHC required.
  • Appropriate work experience in the behavioral health field, with at least five (5) years experience in substance abuse and one (1) year of experience in administering a substance abuse treatment agency.
  • Satisfactory drug screen and criminal background check.
