About This RoleThis is an exciting opportunity to work as a Palliative Care Nurse Practitioner with our team at Empatia, where you will have the chance to make a meaningful difference in the lives of our patients and their loved ones.
You will work collaboratively with our interdisciplinary team to deliver high-quality, patient-centered care that focuses on relief from suffering and improvement in quality of life.
- As a Palliative Care Nurse Practitioner, you will be responsible for assessing patient needs, developing individualized care plans, and implementing evidence-based interventions to address physical, emotional, and spiritual distress.
- You will work closely with our Palliative Medical Director and other team members to ensure seamless coordination of care and optimal patient outcomes.
- Establish relationships with patients and their families, providing empathetic support and education to empower them to take an active role in their care.
- Foster open communication among healthcare providers, family members, and caregivers to ensure comprehensive care planning and collaboration.
To succeed in this role, you will require:
- Excellent leadership, interpersonal, written, and verbal communication skills.
- Independent decision making and self-motivation, with the ability to work effectively in a fast-paced environment.
- A working knowledge or understanding of principles of Palliative Medicine, including symptom management, pain control, and psychosocial support.
- Mandatory continuing education requirements and licensure in good standing.
- The ability to work efficiently with electronic health records and point-of-care software.
You will also need to meet the following minimum qualifications:
- A Master's degree in Nursing from an accredited school of nursing, or an equivalent as allowed under state law.
- A minimum of two years' experience as a Nurse Practitioner in a palliative or supportive care setting.
- Nurse Practitioner specialty in Family or Adult Medicine preferred.
- Practice experience with Part B billing and reimbursement.
- The ability to work autonomously in an Advanced Practice Role, with a focus on patient-centered care.
