Nurse Care Coordinator

We are seeking a skilled and compassionate Nurse Care Coordinator to join our team at Agape Care Group. As a registered nurse, you will play a vital role in providing quality care to patients and their families.

• Evaluate patients and create personalized care plans to ensure the best possible outcomes.
• Communicate effectively with patients, families, and the care team to ensure seamless care coordination.
• Promote patient-centered care by advocating for their needs and preferences.

• Graduate of an accredited school of nursing with a current state license as a registered nurse.
• 2+ years of experience in a clinical care setting, with preference given to those with hospice experience.
• Ability to sit, stand, bend, move intermittently, and lift at least 25 lbs.
