Key Responsibilities- Community Health Outreach: Develop and implement community health outreach programs to engage with individuals and families, assess their health needs, and connect them with appropriate resources and services.
- Health Education and Promotion: Provide culturally competent health education and promotion services to individuals and communities, focusing on disease prevention, health promotion, and wellness.
- Case Management and Navigation: Work with individuals and families to develop and implement personalized care plans, navigate the healthcare system, and access community resources and services.
- Collaboration and Partnerships: Build and maintain partnerships with community-based organizations, healthcare providers, and other stakeholders to ensure seamless coordination of services and resources.
- Data Collection and Analysis: Collect and analyze data on community health needs, program outcomes, and service utilization to inform program development and improvement.
- Program Development and Evaluation: Develop and implement program evaluations to assess the effectiveness of community health programs and services.
- Education: High School Diploma or equivalent required; Bachelor's degree in a related field (e.g., public health, health education, social work) preferred.
- Experience: 2+ years of experience working in community health, healthcare, or a related field; experience working with vulnerable populations preferred.
- Certifications: Promotor(a) or Community Health Worker Certification or willingness to obtain within six months of start.
Full-time position with a flexible work schedule to be determined.
