Job Title: FCHN Case Manager

Special Service for Groups, Inc- HOPICS is seeking a highly motivated and compassionate FCHN Case Manager to join our team. As a key member of our Family Interim Housing Program, you will play a vital role in delivering comprehensive case management and program services to homeless families enrolled in our Family Crisis Housing Program.

Job Summary

The FCHN Case Manager will be responsible for providing case management services to an assigned caseload of homeless families, including engagement, enrollments, assessments, and service planning. This multifaceted role requires strong communication and interpersonal skills, as well as the ability to work effectively with clients from diverse cultural, ethnic, and socio-economic backgrounds.

Key Responsibilities
  • Primary point of contact for families in crisis housing, providing support and guidance to help them achieve self-sufficiency and permanent housing.
  • Complete intakes/enrollments for new participants, including introductions and reviews of program guidelines and consequences for non-compliance.
  • Liaison between Crisis Housing Time Limited Subsidy, Problem Solving, CES Matching, Housing Navigation, and Family Advocate components of the Family Solution Center (FSC) to ensure that families receive appropriate services.
  • Maintain confidentiality in all matters pertaining to participants and their families, developing effective and trusting professional relationships to facilitate provision of services.
  • Assist in making referrals on behalf of families in crisis housing, prioritizing their needs and responding to concerns, complaints, and inquiries in a timely manner.
  • Provide assistance with obtaining necessary program documents, connecting families with mainstream benefits and employment/training opportunities, and accessing necessities such as diapers, feminine products, and childcare.
  • Refer participants to appropriate services, facilitate connection to FSC co-located staff, and transport participants to various appointments, unit viewings, and events as needed.
  • Work with employment services to link participants with services such as employment, education, benefits, health, tutoring, and mentoring as appropriate.
  • Assist in developing successful program exit by producing an exit plan and identifying an exit destination.
Requirements
  • Bachelor's degree from an accredited or state-approved college or university, or two years' experience working in the social service field, with case management and homeless program experience preferred.
  • Working knowledge of Microsoft Word, Excel, and other database programs, as well as knowledge of resource development, case management, and documentation.
  • Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds, and communicate effectively, both written and orally.
  • Verification of Employment Eligibility and Background Clearance, TB test, CPR and First Aid Certification, and valid Driver's License and auto insurance required.
  • Bilingual Spanish/English (both written and verbal) required.
Working Conditions

This position is responsible for working in client-friendly environments and visiting other shelters and homeless access centers as part of their duties. Local automobile travel is required, and some responsibility to work in noisy environments where children and adults are present.

This position will require the individual to be able to handle constant distractions, interruptions, and uncontrollable changes in priorities/work schedules, as well as process information, think, and conceptualize.
