This position is designed for Nurse Practitioners or Physician Assistants who are dedicated to delivering high-quality care in a fast-paced hospital setting.
Key Responsibilities:
- Conduct thorough patient assessments, including comprehensive history and physical examinations, ensuring accurate documentation in medical records.
- Utilize appropriate assessment techniques to gather relevant diagnostic information and perform necessary procedures.
- Identify and evaluate risks associated with the management of acutely ill and complex chronic patients, focusing on medication effects, nutritional status, and fluid balance.
- Collaborate with a multidisciplinary healthcare team to diagnose and manage acute and chronic conditions effectively.
- Oversee diagnostic testing, including ordering, interpreting, and supervising procedures.
- Develop and implement individualized Plans of Care, identifying expected outcomes and addressing complex health needs.
- Apply evidence-based guidelines to create dynamic care plans that adapt to patient needs across various settings.
- Maintain continuous communication with the healthcare team, patients, and families to ensure seamless care transitions.
Documentation:
Ensure that all documentation is timely, compliant with acute care standards, and accurately reflects patient acuity through daily progress notes, transfer summaries, and discharge documentation.
Professional Development:
Demonstrate leadership in professional practice by mentoring graduate students and engaging in educational initiatives.
Qualifications:
- National Board Certification as an Acute Care RNFA Nurse Practitioner or Physician Assistant is required.
