Physical Therapist

**Job Summary:**
We are seeking a highly skilled Physical Therapist to join our team at Prisma Health's Laurens County Medical Campus. As a PT Clinician, you will be responsible for planning and implementing specific treatment programs for individual patients according to principles and practices of physical therapy.
Responsibilities:
- Perform initial evaluations and re-evaluations, and design a physical therapy plan of care.
- Develop and implement comprehensive treatment plans, and participate in patient discharge planning.
- Actively participate in patient and family education process, providing education and training to patients, families, and caregivers regarding diagnoses, progress, and long-term functional outcomes.
- Comply with departmental policies for documentation, ensuring accurate and timely recording of assessment, planning, treatment, and discharge in the medical record.
- Meet team/department productivity standards, and maintain appropriate levels of productivity.
Requirements:
- Bachelor's Degree in Physical Therapy from an APTA accredited school.
- South Carolina Physical Therapy Licensure from the Board of PT Examiners (Lawson Code CLPT).
Estimated Salary Range: $65,000 - $90,000 per year.
