CNA-Health Care Professional

Responsibilities:
Greet and orient new patients to the hospital environment, providing directions and instructions as needed.
Foster patient engagement by involving patients and their significant others in handoff and care planning.
Create a positive patient experience by focusing on what matters most to each patient and visitor.
Proactively meet patient needs and ensure a safe environment through intentional rounding and communication.
Provide ongoing updates to patients and their families through regular communication and updates on the patient-room care board.
Promote a Culture of Safety by integrating Error Prevention tools into daily routines and reporting any abnormal findings.
Assist nursing staff in collecting data for patient assessment and documenting patient progress towards goals.
Work with nursing staff to provide personal care according to organizational standards and policies.
May monitor tube systems, handle medications, update diet orders, and verify tray deliveries meet diet requirements.
May electronically transfer patients in and out of the unit, complete discharge in the EMR, and assist with patient bed placement.
May arrange for patient transport services and enter work orders and obtain supplies from materials.
May enter staff assignments into the Responder 5 System and verify staff have logged in and out of the system.
Qualifications:
High school diploma or equivalent preferred.
No experience required, but SRNA certification in the state of Kentucky and BLS certification within 90 days of employment are required.
Critical thinking skills and decisive judgment are essential for this role.
Ability to work in a stressful environment and take appropriate action.
