We are seeking a skilled Admissions Registered Nurse to join our team at Hospice of Holland Inc. As an Admissions Registered Nurse, you will play a critical role in ensuring that our patients receive the highest quality care and support during their time with us.
Key Responsibilities- Determine patient eligibility for admission into our hospice program through visual contact, interviewing, and medical history review.
- Complete the initial assessment of the admissions process to address patient care needs.
- Explain hospice services, insurance benefits, and complete required paperwork and consents.
- Provide education and support on symptom management and end-of-life care.
- Communicate patient needs to the Registered Nurse Case Manager and on-call staff.
- Consult with physicians for orders and assist with obtaining necessary supplies and services.
- Graduate of an accredited nursing program and current RN licensure in Michigan.
- Two years of recent clinical experience preferred, but recent graduates are also encouraged to apply.
- Homecare or hospice experience desired.
- Sound clinical judgment and knowledge of pain symptom management.
