As an LPN, you will be responsible for coordinating residential health care services, which includes training, monitoring medical requirements, and collaborating with various health care professionals to ensure comprehensive follow-up. Key Responsibilities:
  • Conduct assessments and provide direct nursing care to residents as directed by healthcare professionals.
  • Communicate any medical changes, needs, or concerns regarding residents to the Clinical Vice President, Directors, Senior Nurse Manager, and colleagues through various communication methods.
  • Attend medical appointments as necessary.
  • Coordinate medical support, including emergency treatments for residents.
  • Deliver direct nursing care for all program residents.
  • Train colleagues effectively and ensure their competency in addressing the special needs and medical issues of residents.
  • Provide technical training and ensure colleagues are proficient in using medical devices utilized in the program.
Qualifications:

Essential Requirements:
  • Possession of a Nursing Degree with a valid Massachusetts LPN License in good standing; a minimum of two years of experience is preferred, but recent graduates will be considered.
  • At least one year of relevant experience working with individuals with intellectual and developmental disabilities.
  • Familiarity with MAP regulations and Department of Developmental Services regulations is advantageous.
  • A valid driver's license is required, along with the use of a personal vehicle that meets insurance requirements and satisfactory CORI.
