As a Rehabilitation Nursing Specialist, you will be responsible for orchestrating and delivering comprehensive rehabilitation and restorative nursing care to residents, while also providing oversight and mentorship to clinical team members. This role emphasizes a commitment to enhancing and sustaining optimal physical, mental, and psychosocial well-being. The scope of responsibilities may vary based on state-specific regulations as outlined in The Nurse Practice Act.
KEY RESPONSIBILITIES
The following tasks are typical for this role. This list is not exhaustive and additional duties may be assigned.
- Supervises, guides, and assesses the performance of Restorative Certified Nursing Assistants (CNA's).
- Administers treatments for assigned residents and ensures accurate documentation in compliance with regulatory standards.
- Participates in onboarding and ongoing training programs for staff.
- Initiates and oversees the Rehabilitation/Restorative Functional Maintenance Plan following discharge from skilled therapy.
- Collaborates with the Dietary Manager and Registered Dietitian to implement weight prevention strategies.
- Records and communicates significant observations regarding residents' conditions and any notable changes.
- Coordinates nursing interventions for residents scheduled for therapy or other departmental procedures.
- Assists in emergency situations to provide immediate care for residents experiencing sudden health declines.
- Commences restorative nursing practices when rehabilitation therapy needs arise during custodial care.
- Ensures that the Restorative Certified Nursing Assistant Functional Maintenance Plan aligns with measurable objectives and interventions outlined in the care plan.
- Guarantees that residents have access to necessary equipment to prevent further health deterioration (e.g., positioning aids and splints).
- Maintains a comprehensive understanding of facility policies, procedures, and regulatory requirements.
- Evaluates each new resident for disabilities or weaknesses that may require therapy, as well as those who have recently completed therapy services.
- Identifies residents in need of range of motion exercises.
- Consults with physicians or physical therapists regarding exercise limitations or precautions as necessary.
- Secures physician orders for skilled therapy and ensures prompt implementation.
- Assists in the preparation, execution, and ongoing assessment of residents' care plans, particularly during significant health changes.
- Helps determine individualized care needs for residents.
- Provides emotional support to residents and their families as required.
- Completes the Minimum Data Set (MDS) in accordance with facility guidelines.
- Participates in care plan meetings, Medicare discussions, and rehabilitation team meetings.
- Fosters positive interpersonal relationships and enhances staff morale.
- Ensures adequate inventory and supplies are maintained on the unit and communicates needs for housekeeping, maintenance, and dietary support.
- Responds to resident call lights and ensures regular checks on all residents.
- Reports any suspected abuse to the Director of Nursing or Administrator immediately.
- Performs additional tasks as assigned.

Educational Background and Experience:
- Graduate of an accredited LPN/LVN Nursing Program.
- Possession of a valid LPN/LVN license from the State of Alabama.
- Current CPR certification.
- Preferred: One year of nursing experience.
- Proficient in motivating and directing nursing staff.
- Ability to collaborate effectively as part of a team.
- Strong communication skills with residents, families, and all facility personnel.
- Ability to respond swiftly and effectively in emergency situations.
- Knowledge of training methodologies for clinical staff.
