About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at ResCare Community Living. As a key member of our nursing group, you will provide high-quality care to our clients in a variety of settings, including home health, hospice, and residential care.

Responsibilities
  • Provide individualized care to clients, following their personalized plans of care and promoting their dignity and independence.
  • Monitor clients' vital signs and report any abnormalities to the case manager and physician.
  • Administer medications, feedings, oxygen, and other treatments as needed, and document the results.
  • Collaborate with other healthcare professionals, such as physical therapists and occupational therapists, to ensure comprehensive care.
  • Implement infection control policies and procedures to maintain a safe and healthy environment.
  • Accurately document all nursing actions, communicate with family members and healthcare teams, and report changes in clients' medical conditions.
  • Order supplies and medications as needed to ensure adequate stock.
  • Monitor incidents and ensure timely completion of incident reports.
  • Ensure all medical equipment is properly maintained and report any issues.
Requirements
  • Current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
  • Current Cardiopulmonary resuscitation certification.
  • Ability to work in a home-like setting.
  • Physical ability to bend, stoop, push, pull, sit, and walk for periods of time.
  • Knowledge of individual service plans and client rights.
