Senior Geriatric Physical Therapist

We are seeking a skilled Physical Therapist to join our team in providing top-notch care to seniors in their homes. This is a rewarding opportunity for a motivated professional who is passionate about helping others achieve optimal health and wellness.

As a Home Health Physical Therapist, you will be responsible for delivering personalized therapy services to clients in their homes, promoting independence, and improving quality of life. Our ideal candidate is a licensed physical therapist with at least one year of experience in home healthcare settings.

You will work closely with our interdisciplinary team to develop and implement individualized care plans, conduct assessments, and provide evidence-based interventions. We value teamwork, communication, and compassion in our staff, and we are looking for someone who shares these values.

• Conduct comprehensive assessments and evaluations of clients' physical abilities and limitations
• Develop and implement customized care plans to address clients' needs and goals
• Provide one-on-one therapy sessions, including exercises, stretches, and other interventions
• Collaborate with nurses and other healthcare professionals to ensure seamless care delivery
• Communicate effectively with clients, families, and caregivers to educate and empower them

• Current PA Physical Therapist license
• Minimum one year of experience as a licensed physical therapist in home healthcare settings
• Strong interpersonal and communication skills
• Able to work independently and manage time effectively
• Basic computer skills and familiarity with electronic medical records (EMRs)

• Competitive salary: $80,000 - $110,000 per year
