Position Overview:
The Wound Care RN Case Manager plays a crucial role in providing specialized treatment for patients with various wound conditions.

Key Responsibilities:
- Deliver expert wound care management to patients in outpatient settings.
- Conduct thorough assessments and evaluations of patients' wounds, including but not limited to pressure ulcers, surgical wounds, and diabetic ulcers.
- Create and execute personalized wound care strategies tailored to individual patient needs and medical histories, in collaboration with healthcare providers.
- Administer appropriate wound dressings, medications, and treatments as directed by medical professionals.
- Track and document patient progress in wound healing, making necessary adjustments to treatment plans.
- Work closely with a multidisciplinary healthcare team to ensure comprehensive patient care and optimal healing outcomes.
- Provide education to patients and their families on effective wound care practices, prevention methods, and self-management techniques.

Qualifications:
- Current nursing license in the relevant state.
- Wound care certification (WCC) is preferred.
- Demonstrated experience in delivering wound care services.
- Familiarity with a variety of wound care products, dressings, and treatment options.
- Proficient in using electronic medical records (EMR) for documentation.
- Excellent communication and interpersonal skills for effective interaction with patients, families, and healthcare teams.
- Ability to work autonomously and make informed clinical decisions in a dynamic environment.
- Adaptability to meet the evolving needs and priorities of patients.
