Physical Therapist Home Health Per Diem

We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and surgeries, and regain their independence.
Key Responsibilities- Plan and administer prescribed physical therapy treatment and training for patients to attain the highest level of physical function.
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function and record findings to develop or pursue treatment programs.
- Develop and implement conditioning and rehabilitation programs consistent with physicians' plans of treatment and the overall goals of the patient/rehab team.
- Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising therapy treatment programs.
- Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aide staff, and supervise and evaluate their performance.
- Accurately and thoroughly document patients' care observations, interventions, and evaluations.
- Master's degree from an accredited Physical Therapy Program.
- Minimum of one year of physical therapy experience, with Home Health experience preferred.
- Current and unrestricted Physical Therapy licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
