As a Nurse Practitioner or Physician Assistant on our Senior Community Care team, you'll play a vital role in shaping the future of healthcare.

Primary Responsibilities:
  • Cost-Effective Care Delivery
    • Deliver high-quality, cost-effective care to assigned members
    • Manage medical and behavioral conditions effectively, collaborating with physicians and specialty providers
    • Perform comprehensive assessments and document findings in a concise, compliant manner
  • Care Coordination
    • Understand payer and plan benefits, CCM policies, and articulate them effectively to providers and members
    • Assess medical necessity and effectiveness of ancillary services, initiating benefit events and communicating with providers and team members
    • Coordinate care as members transition through different care levels and settings
  • Program Enhancement
    • Regularly communicate with internal and external parties, including physicians, members, and key decision-makers
    • Actively promote the CCM program in assigned facilities, partnering with stakeholders to maintain and develop membership growth
    • Exhibit original thinking and creativity in developing new approaches to concerns and issues
Requirements:
  • Certified Nurse Practitioner through a national board
  • Active and unrestricted license in the state of residence
  • Current active DEA licensure or ability to obtain post-hire, per state regulations
  • Access to reliable transportation for client and patient site visits
  • Ability to lift a 30-pound bag and navigate stairs and various dwelling conditions
  • Availability to work a minimum of 10 hours per week
Preferred Qualifications:
  • 1+ years of hands-on post-grad experience in Long Term Care
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end-of-life discussions
  • Proficient computer skills, including documentation of medical information
  • Proven ability to develop and maintain positive customer relationships
