Nurse Practitioner or Physician Assistant

As a Nurse Practitioner or Physician Assistant on our Senior Community Care team, you'll play a vital role in shaping the future of healthcare.
Primary Responsibilities:- Cost-Effective Care Delivery
- Deliver high-quality, cost-effective care to assigned members
- Manage medical and behavioral conditions effectively, collaborating with physicians and specialty providers
- Perform comprehensive assessments and document findings in a concise, compliant manner
- Care Coordination
- Understand payer and plan benefits, CCM policies, and articulate them effectively to providers and members
- Assess medical necessity and effectiveness of ancillary services, initiating benefit events and communicating with providers and team members
- Coordinate care as members transition through different care levels and settings
- Program Enhancement
- Regularly communicate with internal and external parties, including physicians, members, and key decision-makers
- Actively promote the CCM program in assigned facilities, partnering with stakeholders to maintain and develop membership growth
- Exhibit original thinking and creativity in developing new approaches to concerns and issues
- Certified Nurse Practitioner through a national board
- Active and unrestricted license in the state of residence
- Current active DEA licensure or ability to obtain post-hire, per state regulations
- Access to reliable transportation for client and patient site visits
- Ability to lift a 30-pound bag and navigate stairs and various dwelling conditions
- Availability to work a minimum of 10 hours per week
- 1+ years of hands-on post-grad experience in Long Term Care
- Understanding of Geriatrics and Chronic Illness
- Understanding of Advanced Illness and end-of-life discussions
- Proficient computer skills, including documentation of medical information
- Proven ability to develop and maintain positive customer relationships
