About the Role

We are seeking a skilled Physical Therapist to join our team in Effingham, IL. As a Physical Therapist with LHC Group, you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their goals.

Responsibilities
  • Evaluate patient care needs and develop treatment plans to improve functional status, endurance, and overall well-being.
  • Provide physical therapy interventions to patients in their homes, including exercises, stretches, and other therapies to promote healing and recovery.
  • Consult with physicians and other healthcare professionals to ensure comprehensive care and coordination.
  • Document patient progress and outcomes, and communicate with the healthcare team to ensure seamless care.
  • Participate in staff conferences and committees to stay up-to-date on best practices and share knowledge with colleagues.
Requirements
  • Current Physical Therapy licensure in the state of Illinois.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance and access to dependable transportation.
