Job Summary:

The Physical Therapist will be responsible for initiating physical therapy programs and instructing other personnel and/or family/caregivers in certain phases of physical therapy. The ideal candidate will have excellent communication skills and be able to work effectively with patients, families, and healthcare teams.


Key Responsibilities:
  • Provides physical therapy services to patients according to a written physician's plan of care.
  • Assesses and evaluates therapeutic/rehabilitative/functional status and participation in the development of the total plan of care.
  • Directs physical therapy treatment and instructs patients/caregivers in the use and care of therapeutic appliances.
  • Determines priority needs for physical therapy and reports to the physician patient's reaction to treatment or changes in condition.
  • May train patients in the use of prosthetic devices and identifies patient and family/caregiver needs for other home health services/hospice and refers as necessary.
  • Prepares and submits clinical and progress summaries based on the attainment of goals.
  • Participates in discharge planning for patients and provides physical therapy consultation to home families/caregivers when indicated.
  • Provides in-service education programs for nursing organization personnel as needed and participates in peer consultation process.

Requirements:
  • BLS/BCLS certifications
  • SNF experience preferred
  • Covid test 72 hours prior to start date with results issued 48 hours prior to start date
