Job Summary: Nirvana Healthcare Management Services is seeking a bilingual Licensed Clinical Social Worker (LCSW) to provide support to our neurology patients. We are looking for a dedicated and compassionate Bilingual LCSW to work in our team, providing comprehensive patient care and ensuring the well-being of our patients through empathetic support and guidance.

Key Responsibilities:
  • Provide counseling and support to patients and families dealing with neurological conditions such as epilepsy, stroke, migraines, and neurodegenerative disorders.
  • Perform assessments to identify patients' emotional, social, and psychological needs.
  • Develop individualized care plans and offer guidance on coping strategies for managing neurological conditions.
  • Collaborate with neurologists, healthcare professionals, and the care team to ensure comprehensive patient care.
  • Educate patients and families on available community resources, support groups, and healthcare options.
  • Assist patients in navigating treatment plans, medication management, and understanding medical diagnoses.
  • Maintain accurate and confidential patient records in compliance with state regulations and organizational policies.
