Job Summary

The Hospital Social Worker is a critical role at MultiCare Health System, Inc., responsible for providing cross-continuum care coordination, psychosocial assessments, and interventions to patients and families.

This position requires a Master's degree in Social Work, licensure in the state of Washington, and a minimum of three years of hospital or equivalent experience.

Key Responsibilities

  • Perform cross-continuum care activities, including clinic, provider practice, urgent care, phone care agency, skilled care, or other sites of care, as appropriate.
  • Provide customer service to patients, families, physicians, and care team members, including discharge milestones and progression to next level of care.
  • Model and orchestrate consistent, professional communications and maintain consistent and proactive communication patterns with colleagues and staff.
  • Delegate responsibilities appropriately but remain accountable for outcomes.
  • Ensure that patients and families experience medically appropriate transitions of care.

Requirements

  • Master's degree in Social Work
  • Licensed in the state of Washington
  • Licensed Independent Clinical Social Worker (LICSW); OR Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain their LICSW within four (4) years
  • Minimum three (3) years Hospital or equivalent experience
  • Health industry knowledge preferred
  • Healthcare financial assistance knowledge preferred
  • Leadership experience a plus
