MDS LPN/RN Role

Responsibilities

Key Responsibilities

  • Coordinate resident assessment and plan of care in accordance with state and federal guidelines
  • Assume authority and responsibility for coordinating MDS, CAAS, and care planning process
  • Complete triple check at beginning of month for all Medicare, managed care, and outpatient therapies
  • Observe and report changes in residents' physical and emotional condition to the unit nurse
  • Manage and update 672/802 and as needed
  • Establish care plan dates within 7 days of completion of MDS or quarterly review
  • Ensure nursing documentation reflects care plan being followed
Requirements

Requirements

  • Graduate of an accredited LPN/RN program
  • LPN/RN license in Illinois
  • 2 years experience in MDS
  • Recent clinical experience, education, and specialty skills specific to geriatrics
  • Flexible schedule, including availability to work evenings, weekends, and holidays as needed
