Caregiver Advocate

About the Role: We are seeking a highly skilled Caregiver Advocate to join our team at Partners Creating Community Inc. As a key member of our staff, you will play a vital role in ensuring the health, safety, and well-being of our residents and contributors.

• Responsibilities:
• Implement individualized support plans to meet the unique needs of each resident/contributor
• Assist with daily living tasks, including meal preparation, hygiene, and medication administration
• Transport residents/contributors to appointments, activities, and community events
• Provide emotional support and guidance to promote personal growth and development
• Maintain accurate records of resident/contributor progress and provide regular updates to supervisors

Requirements: To be successful in this role, you will need to have a high school diploma or equivalent, complete all required training, and possess excellent communication and interpersonal skills. You must also be able to pass a criminal background check and maintain a valid driver's license.
