We are seeking a skilled and compassionate Independent Living Skills Trainer to join our team at RES Home Care, Inc.
Job Summary:
The Independent Living Skills Trainer will work with adults with traumatic brain injury and other physical and/or neurological impairments to assess, train, and supervise them in activities of daily living and instrumental daily living skills.
Key Responsibilities:
- Conduct assessments of ADL/IADL skills and develop comprehensive service plans
- Teach individuals with traumatic brain injury, disabilities, and/or seniors to become more functionally independent
- Collaborate with the multidisciplinary team to ensure seamless care and support
- Document progress and outcomes in a timely and accurate manner
Requirements:
- Dependent on current Department of Health Qualifications
- Minimum requirement is an Associate's Degree with 5 years of experience
- Valid Driver's License
Work Environment:
The Independent Living Skills Trainer will work in the participant's home and in the community, serving clients in Nassau and Suffolk counties.
