At Ensign Therapy, we are seeking a skilled Physical Therapist to join our team at Granite Mesa, a skilled nursing facility providing long term care, post-acute rehab, and geriatric outpatient therapy services.

Key Responsibilities:
  • Effectively screen and evaluate patients with physical functioning disorders and/or wound care, developing an appropriate plan of care in accordance with regulatory and clinical practice standards.
  • Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, adhering to regulatory and clinical practice requirements.
  • Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, wound care, etc.
  • Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
  • Prepare comprehensive discharge summaries of services provided, meeting regulatory and clinical requirements.
