Care Advocate

**Job Overview:**
CN Guidance and Counseling Services is seeking a dedicated Care Advocate to join our team. In this role, you will provide expert support and psychosocial interventions to adults diagnosed with severe mental illness and/or co-occurring substance abuse disorders.
About the Role:
- Conduct assessments of assigned clients to document strengths, needs, goals, and resources.
- Develop and update client-centered comprehensive care plans in consultation with the Program Supervisor and other team members.
- Maintain effective communications with clients, primary care physicians, healthcare providers, family, collateral resources, and Agency staff on behalf of clients.
- Provide telephonic and face-to-face outreach, engagement, and service planning in the field.
Salary and Benefits:
We offer a competitive salary of $65,000 - $75,000 per year, depending on experience, along with an attractive benefits package that includes medical, dental, vision insurance, 403(b) retirement plan, and more.
