About the Role

This is a supportive caregiver role that requires a physical therapy assistant to work with patients, families, and healthcare teams to deliver effective care and improve patient outcomes.

  • As a Home Health Physical Therapy Assistant, you will work with patients to develop and implement personalized care plans that address their unique needs and goals.
  • You will provide exercise instruction, patient education, and support to patients and their families, promoting independence and autonomy.
  • Collaborate with healthcare teams to coordinate care and ensure seamless transitions between settings.

Responsibilities

  • Follow established plans of care and protocols to ensure high-quality patient care.
  • Provide exercise instruction, patient education, and support to patients and their families.
  • Document patient progress and communicate effectively with healthcare teams.

Requirements

  • Current licensure, registration, or certification to assist in the practice of physical therapy in the state of practice.
  • CPR certification is required.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
