Job Title: Contract Liaison

At PACE Southeast Michigan, we are seeking a highly skilled and experienced Contract Liaison to join our team. As a Contract Liaison, you will play a critical role in establishing and maintaining collaborative working relationships between our organization and external stakeholders, including nursing homes, assisted living facilities, group homes, dialysis centers, and contracted providers.

Key Responsibilities:
  • Develop and maintain strong relationships with internal and external staff to ensure effective communication and collaboration.
  • Provide education and training to contracted providers on the PACE model and policy, as well as coordinate communication with internal staff.
  • Conduct monthly walk-throughs at assigned facilities to evaluate participant care and identify areas for improvement.
  • Establish and maintain accurate records of participant data, including MDS scores and other relevant information.
  • Attend IDT meetings and provide updates on participant care and facility concerns.
  • Escalate and document facility concerns to leadership as needed.
Requirements:
  • Current Michigan licensure as a Licensed Practical Nurse (LPN).
  • Experience working in a nursing home setting.
  • Knowledge of MDS and RUGS.
  • Excellent communication and relationship-building skills.
  • Ability to work with diverse populations and teams.
