Job Overview
Northwestern Memorial Healthcare is seeking a dedicated Advanced Practice Provider, specifically a Nurse Practitioner or Physician Assistant, to join our outpatient Primary Care team.
Role Responsibilities
The selected APP will be responsible for managing patient follow-ups and acute visits, providing in-basket support, and assisting physicians with patient care and related tasks.
Work Schedule
This position operates Monday through Friday during daytime hours, with occasional on-call responsibilities during the week and potential overnight coverage, excluding weekends and holidays.
Key Responsibilities
1. Conduct thorough evaluations of patients' health by obtaining medical histories and performing physical assessments.
2. Deliver direct clinical care and prescribe medications and therapies as per clinical privileges.
3. Order and interpret diagnostic tests, including laboratory work and imaging studies.
4. Assess patients' needs for additional healthcare services and provide comprehensive education to patients and their families.
Professional Development
Maintain a high standard of care by staying current with certifications and continuing education requirements. Serve as a mentor to graduate students and actively engage in quality improvement initiatives and research relevant to the patient population.
Advocacy and Collaboration
Act as a patient advocate, ensuring the highest level of care and support for patients, families, and colleagues. Collaborate with other healthcare professionals to enhance patient outcomes and contribute to departmental committees and task forces.
