We are looking for a Caregiver Liaison - Community Outreach to join our team. This role will involve providing supportive care to identified Healthy Families clients and their families in the home or in the community.

The scope of this position includes assisting with direct care, coordination of care, client/family education, outreach, and advocacy. The successful candidate will have a strong understanding of child and family development and prior experience working with children and families.

Responsibilities:

  • Provide direct care and support to Healthy Families clients and their families
  • Coordinate care and services to meet the needs of clients and their families
  • Educate clients and their families on healthy habits and wellness practices
  • Promote outreach and advocacy for Healthy Families clients and their families

Requirements:

  • High School diploma, GED or equivalent required; higher education recommended
  • Experience in child and family development and working with children and families
  • Bilingual in English and another language preferred
