Job Summary

We are seeking a skilled Nurse Case Manager to join our team. As a key member of our interdisciplinary team, you will provide professional nursing care to patients in their homes, ensuring seamless coordination of care and optimal patient outcomes.

Responsibilities
  • Plan, provide, and document professional nursing care for patients in accordance with nursing practices, agency policies, and regulations.
  • Collaborate with healthcare teams to develop and implement individualized care plans, ensuring patient-centered care and effective communication.
  • Conduct comprehensive assessments, identify patient needs, and develop strategies to address them.
  • Work closely with patients, families, and caregivers to educate and empower them to manage their care.
  • Participate in interdisciplinary team meetings and case conferences to ensure comprehensive care and service delivery.
Requirements
  • Current RN license in NC or compact RN licensure from participating state.
  • Valid driver's license and current automobile insurance coverage.
  • 1-2 years of med-surgical, critical care, or home health/hospice/infusion experience preferred.
  • Knowledge of Medicare and Medicaid regulations, including JCAHO and DFS standards.
