Job Description

We are seeking a highly skilled Nursing Professional to join our team as a Patient Care Specialist at DHR Health. This is a fantastic opportunity for an experienced nurse to take their career to the next level and work in a dynamic environment.

About the Role

As a Patient Care Specialist, you will be responsible for delivering high-quality patient care through the nursing process of assessment, diagnosis, planning, implementation, and evaluation. You will work closely with interdisciplinary teams to ensure effective and efficient patient care delivery and achieve desired patient outcomes.

Responsibilities
  • Ensure that care is delivered to each patient in accordance with the Nurse Practice Act of the State of Texas and ANA Standards of Clinical Nursing Practice.
  • Perform family-centered evidence-based nursing care through the nursing process of assessment, diagnosis, planning, implementation, and evaluation.
  • Implement physician's orders in a safe and accurate manner.
  • Assist with admitting, discharging, and transferring patients.
  • Transcribe and implement new physician orders.
  • Adapt to the needs of the unit by demonstrating flexibility and adaptability.
  • Maintain detailed and accurate records of nursing actions.
  • Participate in health/therapeutic counseling, teaching/emotional support for patients and significant others.
  • Makes rounds on assigned patients and prioritizes patient needs.
  • Maintain a safe and clean environment for patients and co-workers.
  • Provides and receives reports in a status of patients at the change of shifts.
  • Administer medication and therapies within the scope of safe nursing practice.
  • Formulate, modify, and implement goal-oriented patient care plans according to facility policies.
  • Assume responsibility to report clinical limitations or the need for assistance to the supervisor.
  • Perform other duties as assigned.
  • Abide by the rules and regulations set forth by the Board of Nurse Examiners.
  • Adhere to policies and procedures that guide and support the provision of services.
  • Inform the charge nurse of any anticipated and/or actual issues/problems.
  • Maintain current with all new policies and procedures published.
  • Provide direction to non-licensed nursing staff in the delivery of patient care.
  • Anticipate potential problems situations and intervene to offset any adverse impact.
  • Contact the organ procurement organization in order to satisfy regulatory requirements for the determination of medical suitability for organ, tissue, and eye donation; complete certificate of referral/request and place same in the medical record.
  • Build a positive work environment by guiding employees to support the mission of the hospital, providing developmental opportunities, and encouraging teamwork.
  • Ensure the distribution and delivery of equipment/supplies as necessary.
  • Flexibility in schedule to meet unit needs and weekend/holiday requirements.
  • Plan and coordinate time schedule with peers to provide coverage.
  • Participate in unit-based council and/or hospital committees.
  • Attend mandatory unit and hospital meetings/classes.
  • Ensure care is delivered to each patient in accordance with the Nurse Practice Act of the State of Texas and ANA Standards of Clinical Nursing Practice.
Estimated Salary: $65,000 - $80,000 per yearRequirements
  • Hospital experience preferred.
  • Proficiency in English (verbal and written) and computer skills required.
  • Spanish proficiency preferred.
  • Able to practice nursing as described by the definition by the American Nurses Association (2016).
  • Able to utilize the best nursing process in the provision of nursing care including but not limited to administering treatments and medications, interpreting diagnostic tests, formulating a plan of care, collaborating with other healthcare providers, and providing education to patients and/or significant others.
