Job Title: RN Case Manager Liaison

Kaiser Permanente is seeking a skilled Registered Nurse (RN) to join our team as a Case Manager Liaison. This role is a critical part of our healthcare team, responsible for ensuring the seamless transition of patients from hospital to home.

Job Summary:

The RN Case Manager Liaison will work closely with patients, families, and healthcare providers to develop and implement individualized care plans. This role requires strong communication and collaboration skills, as well as the ability to navigate complex healthcare systems.

Key Responsibilities:
  • Assess patients' physical, mental, and financial needs to identify existing and potential needs.
  • Develop and implement patient-centered care plans, including goals, objectives, and outcomes.
  • Collaborate with healthcare providers to ensure timely and effective care coordination.
  • Monitor and evaluate patient responses to care interventions and make adjustments as needed.
  • Document all interactions and interventions in accordance with Kaiser Permanente policies and procedures.
Requirements:
  • Minimum three (3) years of recent RN medical/surgical/ambulatory clinical experience.
  • Bachelor's degree or Bachelor's of Science in Nursing.
  • Effective, independent nursing judgment and skills, and use of evidence-based clinical decision-making criteria.
  • Knowledge in management of chronic disease process, nursing process, and collaborative care planning.
